How to Schedule an Appointment at Revolution Health

At Revolution Health, we make scheduling your appointments as simple and convenient as possible through our online Patient Portal. Whether you’re booking a new patient consultation, a follow-up visit, or lab testing, you can request your appointment in just a few steps.

Below is a quick guide on how to schedule your next appointment:


Step 1: Log in to Your Patient Portal

Start by logging in to your secure Patient Portal. Once logged in, go to the “My Appointments” section.


Step 2: Select Your Provider

Choose the provider you’d like to see. This depends on your needs—whether you’re scheduling a hormone consultation, a prolotherapy visit, or a wellness check.

  • Tip: If you’re scheduling a lab draw or test, select “Labs & Testing” as your provider option.


Step 3: Select the Appointment Type

Next, select the appointment type. Examples include:

  • New Patient Visit

  • Follow-Up Visit

  • Telemedicine Appointment

  • Lab/Testing Appointment


Step 4: Pick Your Preferred Date

Browse the calendar and select the date that works best for you.


Step 5: Choose a Time Slot

Available times will appear as blue boxes. Click the time that best fits your schedule.


Step 6: Add Comments or Specific Requests

Here’s your chance to let us know why you’re scheduling. Use the comments box to describe the reason for your visit or include any special requests.

  • Examples: “Follow-up on lab results” or “New knee pain – prolotherapy evaluation.”


Step 7: Submit Your Request

Click “Request Appointment.”

You’ll immediately see a notification that says “Pending Approval” along with your appointment details. This means your request has been received.


Step 8: Receive Confirmation

Once our staff approves your appointment, you’ll receive an email notification confirming the date, time, and provider. At that point, your appointment is officially scheduled.
