We begin with a comprehensive intake and in-person evaluation, followed by a structured care plan and ongoing follow-up. This approach allows us to provide thoughtful, appropriate care while maintaining clear expectations from the start.
All new patients begin by completing a secure online intake. This helps our team understand your health history, goals, and current concerns before your first visit.
Your initial visit is conducted in person at our clinic. This allows for a thorough evaluation and ensures care is safe, appropriate, and individualized.
Based on your visit, we develop a care plan tailored to your needs. Recommendations may include lifestyle strategies, medical therapies, procedures, and structured follow-up.
Care at Revolution Health is ongoing. Follow-up visits, care coordination, and monitoring help ensure your plan remains effective and appropriate over time.
Some patients may benefit from in-person procedures such as pellet therapy or regenerative treatments. These follow defined care pathways and are discussed individually.
To ensure your visit is productive, all new patients complete their intake before scheduling an appointment.
Once your intake is submitted, you’ll receive access to your patient portal with instructions for scheduling your initial visit through our online system.
Revolution Health is not designed for one-time consultations outside of specific procedures. Our care models emphasize continuity, follow-up, and long-term health outcomes.
Ongoing care may include regular check-ins, coordination between visits, and monitoring to support safety and effectiveness. These elements are part of how we deliver comprehensive care across our practice.
Some patients receive care through Medicare-Supported Care models. These models include structured physician oversight, care coordination, and physiologic monitoring.
Participation is discussed as part of your individualized care plan and requires patient consent. When clinically appropriate, Medicare may cover portions of these services.
Telemedicine is frequently used for established patients who have been seen in person.
After an in-person relationship is established, virtual visits may be used for follow-up, care coordination, and ongoing management when clinically appropriate.
Revolution Health is not a telemedicine-only practice. Patients must be able to receive in-person care at our clinic.
