  1. Visit the Revolution Health website at RevolutionHealth.org.

  2. Click on the “Patient Portal” link (found in the navigation bar or homepage).

  3. Enter your username and password.

    • If you don’t yet have an account, call our office and we’ll help you get set up.

    • If you’ve forgotten your password, click “Forgot Password” to reset.

Once logged in, you’ll be taken to your dashboard, the homepage of your Patient Portal.

  • View upcoming and past appointments

  7. Click “Request Appointment.”

You’ll see a notification that says “Pending Approval.” Once our team approves your request, you’ll get a confirmation email.


Requesting Medication Refills

Managing refills is simple with the portal:

  1. Go to “My Pharmacies.” Make sure your preferred pharmacy is listed and set as Primary Pharmacy.

  2. Next, go to “My Medications.”

  3. Find the medication you need refilled.

  4. Click “Request Refill.”

  5. Add any comments or instructions (like “90-day supply if available”).

  6. Submit your request.

You’ll see “Pending requests awaiting review” and a “!” icon next to the requested medication. Once approved, it will be sent to your pharmacy.


Viewing Lab Results

  1. Click on the “Labs/Results” tab.

  2. Select the test you’d like to view.
