The REV Membership replaces the traditional Direct Primary Care model with a smarter, flexible structure that bills insurance where possible and gives you control over your care.

Whether you want full-spectrum functional support or just need a trusted clinic, we have a REV tier that fits.

How It Works

  • We bill your insurance for office visits, labs, and other covered services

  • Your membership supports what insurance doesn’t cover: deeper testing, technology platforms, personalized care, and your annual assessment

  • You choose your tier—based on how deep you want to go

Choose Your Membership Level

REV Access – $540 (Annual Membership)

Functional Foundations access + technology support
Ideal for those who want clinic access, basic support, and accountability.

Includes:

  • Unlimited clinic access

  • Visits billed to insurance (or 3 included if you’re paying cash)

  • Additional visits at a discounted rate

  • Remote Physiologic Monitoring (RPM) platform access (e.g., Withings, Oura)

  • No fees for missed appointments, refill requests, or forms

Does not include:
Annual assessment, advanced testing, discounts on services

REV Core – $1440-1920 (Annual Membership) based on age

Our most popular tier.
The foundation of your personalized prevention plan, backed by advanced diagnostics and extended visits.

Includes everything in REV Access, plus:

  • Annual Revolution Health Assessment: includes services such as DEXA scan (bone density &/or body composition), EKG, SphygmoCor, and others. 

  • Extended visit times (30–60 minutes as needed)

  • Unlimited appointments billed through insurance

  • RPM platform access

  • No refill/no-show/form fees

  • Discounts on:

    • Supplements

    • Peptide therapy

    • In-house services

The Annual Fee for REV Core is based on age as follows:

  1. 18-39 years of age: $1440
  2. 40-64 years of age: $1680
  3. >65 years of age: $1920

REV Elite – $xxxx (Annual Membership)

The ultimate membership for total prevention and longevity.
Designed for high-risk individuals, executives, or those ready to go deeper.

Includes everything in REV Core, plus:

  • PULS Test ($299 value) – predicts 5-year heart attack risk

  • CIMT (Carotid Intima-Media Thickness) ($250 value) – detects early arterial plaque

  • CardiaX ($450 value) – advanced cardiovascular genetics + functional biomarkers

  • EndoPAT testing ($175 value) (if available) – assesses endothelial function

  • Galleri - annual cancer screening ($1050 value)

  • Priority scheduling + concierge-level care planning

What About Labs and Insurance?

We bill your insurance for all applicable services—office visits, labs, diagnostics, and procedures.
If you have BCBS or your insurance doesn't cover your labs, you’ll receive access to our discounted cash-pay lab pricing.

Included in your membership:

  • Non-covered diagnostics like DEXA, central BP, RPM platform access

  • Select advanced cardiovascular tests in REV Elite
