Step 1: Confirm You Live in Oklahoma

First things first—we can only accept new patients who live in Oklahoma.

If you live outside of Oklahoma, we are happy to see you but you'll need to come to our clinic in Sapulpa, OK.

Step 2: Choose Your Membership

REV Membership is our innovative approach to healthcare that goes beyond traditional primary care. Instead of quick, transactional visits, REV Membership creates a long-term health partnership focused on prevention, advanced diagnostics, and functional medicine. REV Memberships allow you to utilize your insurance but maintain the higher levels of care you get with a cash practice while saving you money.

Insurance billing applies to eligible services. Membership fees cover non-insurance services and program benefits.

Step 3: Register as a New Patient

The next step is to fill out the Registration Form. The more thorough you are in filling out the form, the easier the process will be once you become a patient.

Once you click '+Sign Me Up' at the bottom of the form, you'll receive an email with a title of 'Please Confirm Your Registration'. Follow the prompts to create your Patient Portal access. 

Step 4: Complete My Account Details

We need your basic information to build your chart and verify your identity.

  1. Enter your address, city, and state.
  2. Add your race, ethnicity, and pronouns.
  3. Save your account details and set your contact preferences.
  4. Upload a current photo.
  5. Add your insurance information and upload photos of your insurance card.

Step 5: Add Payment Method

To reserve your place as a patient, a one-time retainer is required.

  1. Open Invoices & Payment.
  2. Add your credit card information and save it.

You can pay any account balances in this section of your portal as well.

Step 6: Complete Forms & Questionnaires

These forms help us understand your health history and current needs.

  1. Go to Forms & Questionnaires in the portal.
  2. Fill out everything marked with a red Start+.
  3. Please be thorough—especially with the Medical Intake Form—so we can provide the best possible care.

Step 7: Add Your Vitals

Tracking vitals helps us measure progress and adjust care as needed.

Withings devices integrate seamlessly with our system so we can track your progress. Recommended devices:

  • Blood Pressure Cuff
  • Body Smart Scale or Body Comp Scale

Enter your blood pressure, heart rate, height, and weight in the portal under My Vitals.

Step 8: List Your Medications

This ensures your provider has an accurate picture of what you’re currently taking.

  • Go to My Medications.
  • If you don’t take medications, check “I am not currently taking any medications.”
  • Otherwise, add each medication by searching, selecting, and saving.

Step 9: List Your Supplements

Knowing your supplements is AS important as knowing your supplements.

  • Go to 'My Supplements'.
  • If you don’t take supplements, check “I am not currently taking any supplements.”
  • Otherwise, add each supplement by searching, selecting, and saving.

Step 10: Add Your Preferred Pharmacy

Even though we dispense many therapies directly, a pharmacy on file is still required.

Step 11: Schedule Your New Patient Appointment

Once Steps 1-10 above are complete, you'll be able to schedule an appointment.

  • Go to My Appointments.
  • Select the provider you'd like to see
  • Select either 'New Patient' or 'New Patient 2' for the appointment type
  • Select your preferred date. If you don't find an appointment on the date of your choice, be sure to check the other appointment type as noted above.
  • Select the Start Time
  • Describe why you want to be seen and click 'Request Appointment'
  • You'll see an Upcoming Appointment "Pending Approval"
