How to Become a New Patient:

At Revolution Health & Wellness, we’re transforming the way healthcare works, making it more personal, proactive, and prevention-focused. The only way to become a patient is through our REV Membership program, which gives you unlimited access, advanced diagnostics, and a dedicated care team committed to your long-term wellness.

Step 2: Choose Your Membership

REV Membership is our innovative approach to healthcare that goes beyond traditional primary care. Instead of quick, transactional visits, REV Membership creates a long-term health partnership focused on prevention, advanced diagnostics, and functional medicine. REV Memberships allow you to utilize your insurance but maintain the higher levels of care you get with a cash practice while saving you money.

What Happens Next

Be sure to fill out the entire form. Once you complete the form and click '+SIGN ME UP!' you'll receive an email with a title of Please Confirm Your Registration. Follow the prompts to create your Patient Portal access. 

Once Logged In: Complete Your Portal Setup

➤ Click on My Account Details

  1. Click ‘My Account Details’ (right side of screen)
    • Enter your home address, city, and state

    • Enter your race, ethnicity, and pronouns

    • Click ‘Save account details’

  2. Select Contact Preferences → click ‘Save Contact Preferences’

  3. Upload a current photo

  4. Enter your Insurance Information → be sure to click 'SAVE INSURANCE INFORMATION'

  5. Upload Insurance Card Photos

➤ Click on Invoices & Payment

  1. Click ‘Add Credit Card’
  2. Complete all fields and click ‘Save Card’
  3. If there is a Red bar at the top of the page showing "Current Balance: -$200.00" - this is the Retainer that must be paid before appointments can be scheduled. This Retainer is non-refundable but will apply to all services rendered in the clinic. 
  4. Click 'Make Payment' on the Right and follow the prompts. 

➤ Click on Forms & Questionnaires

  1. Complete all forms marked with a red ‘Start+’ box
  2. The more detailed you are in filling out the information, specifically in the Medical Intake Form, the more accurate we can be in helping to serve you. 

➤ Click on My Vitals

We highly recommend obtaining a Withings Blood Pressure Cuff and a Withings Scale. These devices integrate with our systems allowing us to follow along with your progress in a very simplistic manner. We have these devices available in the clinic (where we can save you some money) or you can purchase them on Amazon:

  • Blood Pressure Cuff
  • Body Smart Scale or Body Comp Scale
  • Go to ‘My Vitals’ → click ‘Add a blood pressure reading’
    • Enter:
      • Systolic (top number)

      • Diastolic (bottom number)

      • Heart rate

      • Date

    • Click ‘Submit this reading’

  • Add your current Height and Weight 

➤ Click My Medications 

  1. Click the box "I am not currently taking any medications" IF you aren't taking ANY medications.
  2. Otherwise, click 'ADD A NEW MEDICATION+' 
  3. Type the name of your medication to search for it and click on the appropriate result
  4. Add any notes about this medication
  5. Click 'COMPLETE AND ADD THIS MEDICATION+'
  6. Repeat this process for each additional medication

➤ Click on My Supplements

  1. Repeat the process listed above under My Medications

➤ Click on My Pharmacy

It is essential that we have a pharmacy on file for you. While we may be providing your supplements, peptides, and even medications, situations may arise that require a prescription to be sent. Having a pharmacy in your chart will save time and help us serve you faster and more reliably. 

  1. Go to ‘My Pharmacies’ → add your preferred pharmacy
