Complete Your New Patient Chart

Completing your new patient chart is an important step in preparing for care at Revolution Health. This information allows our clinical team to review your history ahead of time and ensures your first visit is appropriate, efficient, and productive.

This guide walks you step by step through each section of the chart and explains what is required, what is optional, and how to avoid common issues.

If you follow this guide while completing your chart, most patients are able to finish in one sitting.

Before You Start

Before logging in to complete your chart, please keep the following in mind:

  • Use a desktop or laptop computer if possible for the best experience
  • Allow enough uninterrupted time to complete all required sections
  • Have a list of your medications, supplements, and prior diagnoses available
  • Save your progress as you go

Your chart is considered complete only when all required sections are submitted.

How to Access the Portal

  1. Log in to the patient portal using the email address you used during registration
  2. Use the password you created during registration
  3. If you forgot your password, use the “Forgot Password” option on the login page
  4. Once logged in, navigate to your New Patient Chart or Forms section

If you do not see your chart immediately after logging in, try refreshing the page or logging out and back in.

Profile & Demographics

This section confirms basic information used for identification and communication.

Please complete all required fields, including:

  • Legal name
  • Date of birth
  • Address
  • Phone number
  • Email address

Make sure your contact information is accurate, as portal messages and scheduling instructions are sent through this system.

Medical History

This section helps your provider understand your past and current health concerns.

Please include:

  • Current medical conditions
  • Past diagnoses
  • Surgeries or hospitalizations
  • Significant illnesses or injuries

If you are unsure whether something is relevant, include it. It is better to provide more information than too little.

Medications & Supplements

Please list all current medications and supplements, including:

  • Prescription medications
  • Over-the-counter medications
  • Vitamins and supplements
  • Hormones or injections

For each item, include the name, dose, and how often you take it if known. If you no longer take a medication, do not include it.

Allergies & Reactions

List any known allergies or adverse reactions, including:

  • Medication allergies
  • Food allergies
  • Environmental allergies
  • Reactions to medications or injections

If you have no known allergies, indicate this clearly if the option is available.

Family History

Family history helps assess inherited risk factors.

Please include known conditions in immediate family members, such as:

  • Heart disease
  • Diabetes
  • Autoimmune conditions
  • Cancer
  • Thyroid disorders

If information is unknown, you may leave those fields blank or indicate “unknown.”

Social & Lifestyle History

This section provides context for your health.

You may be asked about:

  • Tobacco or nicotine use
  • Alcohol use
  • Exercise habits
  • Sleep patterns
  • Stress levels

Please answer honestly. This information is used to guide care, not for judgment.

Uploading Records or Documents

If you are asked to upload documents, you may include:

  • Recent lab results
  • Imaging reports
  • Prior medical records
  • Relevant outside evaluations

Upload only documents that are relevant to your current care. If you do not have records available, you may proceed without uploading files.

Consents & Acknowledgments

These forms are required before scheduling and may include:

  • Treatment consent
  • Telemedicine acknowledgment
  • Privacy and HIPAA notices
  • Financial or membership acknowledgments

Please read each consent carefully before signing. If a required consent is not completed, your chart will remain incomplete.

I can’t see my forms:

Try logging out and logging back in, or refresh the page.

The portal won’t save my answers:

Ensure all required fields are completed before saving.

I didn’t receive the registration or confirmation email:

Check your spam or junk folder and ensure you used the correct email address.

I’m not sure how to answer a question:

Provide your best estimate or explanation. Your provider can clarify during your visit.

After You Finish Your Chart

Once your chart is complete:

  • Our clinical team will review your information
  • You will receive instructions through the patient portal regarding scheduling
  • Any additional requirements will be communicated before scheduling

Please note that scheduling instructions are sent after chart completion.
