Your completed chart allows our team to confirm the most appropriate pathway based on your needs and goals.

Membership & Procedural Requirements

Some care models require membership enrollment, while others follow defined procedural pathways. These details are reviewed after chart completion and prior to scheduling to ensure expectations are clear.

Medicare-Supported Care

Patients with Medicare may receive care through Medicare-Supported Care models that include physician oversight, care coordination, and monitoring. Participation is discussed as part of an individualized care plan and requires patient consent.

Visit Format

Initial visits are conducted in person at our clinic.

After an in-person relationship is established, telemedicine may be used for follow-up and ongoing care when clinically appropriate.
