This is the one-time $200 enrollment fee for Male Pellet Membership.
Use this if you already know you want bioidentical hormone pellets. This is not a consultation and it does not start treatment. After you purchase, our team contacts you within one business day to complete your chart, the pellet agreement, and the next clinical steps (labs and insertion when appropriate).
Membership is $150 per month after enrollment. Insertions are included when they are medically appropriate. There is no per-insertion fee. Cancel anytime with 30-day notice. We do not bill insurance for the membership or the insertion procedure.

Do not buy this. Book the $99 Male Pellet Consultation instead. If you join after that visit, the $99 is credited toward this enrollment fee.

What this does not include
Monthly membership dues, laboratory testing, REV Core / REV Access care, or a guaranteed insertion on a set date. Pellets are placed when they are clinically appropriate.
