REV Core Enrollment

This is the one-time $200 setup fee for REV Core, our full functional-medicine membership.

Use this if you already know Core is the right fit. This is not a medical visit and it does not start treatment. After you purchase, our team contacts you within one business day to complete your chart, the Core agreement, membership billing, and schedule your first visit.

Annual or monthly dues are separate and are based on age and/or detailed in the membership agreement. Labs are billed to insurance when we can. Cash and insurance members have included visits each year, then member pricing after that.

Not sure Core is the right size?
Do not buy this. Book the $99 Pathway Consult instead. That visit is brief and only helps you choose a path. It is not a treatment visit.

Traditional Medicare (Part B ± a supplement we can bill)
Do not buy this. Setup is waived. Call 918-935-3636 or use the Medicare path on the membership page. 
CommunityCare Senior and CommunityCare commercial are cash — they are not the Medicare membership.

Want pellets?
Do not buy this. Use Female or Male Pellet Enrollment.

What this does not include
Monthly or annual membership dues, office-visit fees beyond what Core includes, laboratory testing, Pellet Membership, or REV Access.
